Our Flight Request Process

Children’s Flight of Hope works to make the request process as easy as possible for our families. There are three eligibility requirements: a signed waiver, medical approval from the child’s physician, and a financial need verification from a non-family member. See the requirements information below for more details.

Once a request is submitted and all documentation requirements are met, a CFOH Mission Delivery Specialist will connect with you directly to schedule your flight.

How to Request a Flight

Returning Client

If you are an existing CFOH client, please use the returning client form here to complete your request. You will need to have the child’s Client ID and their Date of Birth to begin the application. The Client ID is an 8-digit alphanumeric code created specifically for each child and is used to recall previously submitted information. If you need to access the Client ID, please email missions@cfoh.org and a Mission Delivery Specialist will assist.

American Airlines & International Requests

If you have been referred to CFOH directly through American Airlines or one of their many approved partner organizations to request air transportation assistance, click here to begin the online request, and answer the qualifying questions accordingly.

New Clients:

Due to overwhelming requests for air transportation assistance, and our commitment to providing ongoing support to children who are currently enrolled in our program, Children’s Flight of Hope is not accepting new clients at this time. Please check back periodically as we hope to reopen enrollment in the near future.   

Exception: If you have been referred to Children’s Flight of Hope directly through the American Airlines Miles Well-Being Program, please move forward with this online form by answering ‘No’ to the question below and then clicking ‘Next Page’.

Our Requirements

Time Requirements

Requests must be submitted at least two weeks prior to the preferred departure date in order to allow for enough time to process the request. Requests not submitted with enough notice may not be able to be fulfilled.

Age Requirements

CFOH provides services to children between birth and their 18th birthday.

The child’s travel companion (preferably the child’s legal guardian) must be 18 or older.

Medical Need

Children’s Flight of Hope works directly with the destination or referring physician to obtain medical approval for travel. The child’s treatment being sought must save, prolong, or enable a better quality of life for the child. Flights will not be booked unless the child has a confirmed appointment.

Financial Need

Children’s Flight of Hope is able to assist families when the cost of air transportation will prohibit travel or place a significant financial hardship on the family. Financial verification can be acquired through proof of government subsidy for the child or family or through a social worker or other relevant professional completing and returning a Children’s Flight of Hope financial verification form.

Service Area

Children’s Flight of Hope serves families who live in and are traveling to medical facilities within the United States. Through our partnership with American Airlines, CFOH is able to provide international flights to children needing medical treatment in the United States or Canada who have been referred by partnering organizations.
